A Special Needs Plan (SNP) is a type of Medicare Advantage plan that combines all the benefits of Original Medicare (Parts A and B) with prescription drug coverage (Part D). There are 3 types of SNPs, including Chronic Special Needs Plans (C-SNPs), Institutional Special Needs Plans (I-SNPs), and Dual-eligible Special Needs Plans (D-SNPs), all designed to meet different types of special needs. SNPs are only available to people who have an additional qualifying condition, are a resident of a participating long-term care facility or are eligible for both Medicare and Medicaid. Many Humana SNP plans include dental, vision or hearing benefits.

  • Works with Medicaid
    D-SNP

    Humana D-SNPs offer coverage that works well with Medicaid and most plans include a Humana Healthy Options Allowance™ that can be used to pay for eligible living expenses.

    • For members eligible for both Medicare and Medicaid
    • Expanding to 45 counties in 5 states: Maryland, Missouri, Montana, North Dakota and Washington
    • Can be sold by any agent contracted to sell Medicare

  • Bringing care to long-term care facilities
    I-SNP

    I-SNPs are specially designed for residents of long-term care facilities and focus on onsite, coordinated and dedicated care and other special needs of residents.

    • For Medicare beneficiaries eligible for Part A, enrolled in Part B and residing in a participating long-term care facility
    • Available in Iowa, Kentucky, Louisiana, New Mexico, Tennessee, Washington, West Virginia, Georgia, Indiana, Ohio, South Carolina, Texas and Virginia
    • Can only be sold by Longevity agents, can be sold year-round

  • Helps care for chronic conditions
    C-SNP

    C-SNPs offer extra support to people with an eligible chronic or disabling condition such as diabetes, chronic heart failure, chronic lung disease or end-stage renal disease.

    • For people enrolled in Medicare Parts A and B with a doctor-verified, qualifying health condition
    • Expanding to 280 counties in 9 states: Arizona, Arkansas, Illinois, Indiana, Kentucky, Michigan, Minnesota, New Mexico and Texas
    • group-large Can be sold year-round by any agent contracted to sell Medicare
